Insurance and Other Forms of Payment


Private/Group Health Insurance
East & West Physical Therapy is a preferred provider for the majority of group health insurance plans. We are pleased to assist in the process of determining what your insurance policy will cover. In order to do so, we request detailed information about the patient’s insurance eligibility and benefits prior to the patient’s first visit. Please be prepared to provide an insurance identification card, which should include the group policy number, the policy holder’s name, and if the patient is not the policy holder, we will require information about the patient’s date of birth. After receiving this data, each patient may be properly informed of how much insurance will cover to help meet their physical therapy needs.

We provide insurance billing services for our patients on a monthly basis. Any balance due after the patient’s insurance has paid its portion, such as coinsurance and/or deductible amounts, is billed to the patient on a monthly basis. Co-payments are collected at the time of each visit. Patients whose annual deductible exceeds $200.00 are asked to pay for their visits in full until their deductible has been met.

Auto Insurance
Patients who are involved in a motor vehicle accident may choose to use their automobile insurance if there is a “Medical Pay” section included in their policy. These amounts generally range from $1,000.00 to $20,000.00 depending on the policy, and our clinic can bill the patient’s insurance up to the amount of “Medical Pay” included in their policy.

Self-Pay
We have affordable payment options for those patients who are uninsured or who have high deductible plans or excessive copays.

We accept payments by cash, check, MasterCard, Visa Debit and/or Credit card, and DiscoverCard.
